For Individuals with Chronic Illness & Caregivers

Community Resource Referral

Clinician will provide community resource referrals during part of the diagnostic assessment or counseling sessions as necessary, depending on patient's individual needs. Clinician will often use a part of the counseling session to provide patient education on how to navigate necessary community resources to address client's needs. Patients are encouraged to inquire with clinician about any needs for assistance throughout treatment.

Chronic Illness Medication Assistance Consultation

Consultation with clinician to review patient's individual medication financial needs, review physician medication list, and assist patient with applying for free and low cost medication programs and grants. Clinician also provides education to patient on how to navigate low-cost options for prescription medications in the future.

Advocacy

Clinician will advocate for patients, both by helping patients to address their mental health needs and by acknowledging any cognitive or physical issues related to illness. Clinician will help empower patients through ongoing advocacy as necessary. Patients with chronic illness and disability often experience issues in the work, school, or community setting, which require accommodations. Clinician can help patients understand their basic rights, navigate disability accommodations, and provide referral to legal assistance or community agencies as necessary for continued advocacy and support.

Caregiver Support & Counseling

Family members of individuals with mental health issues and caregivers of adults or children with chronic illness sometimes need their own support. Too often, parents and caregivers do not address their own mental health concerns or seek out support to discuss the emotional issues that can come along with the stress of providing care to a loved one. Many caregivers I have worked with admit to experiencing stress, but lacking time for their own appointments due to providing 24/7 care to their loved one. Virtual counseling is excellent for caregivers needing support from a clinician experienced working with patients and families with various mental health issues and chronic illnesses.
